Reports To: Registered Nurse (RN) / Nursing Supervisor

Job Summary

The Licensed Practical Nurse (LPN) provides skilled nursing care under the supervision of a Registered Nurse (RN) or physician. LPNs play a critical role in supporting patient care by administering treatments, monitoring health status, and ensuring patients’ comfort and safety in long-term care facilities, rehabilitation centers, assisted living communities, and home health environments.

Key Responsibilities

  • Provide direct patient care including administering medications, wound care, and treatments as prescribed.

  • Monitor, document, and report patient health conditions, vital signs, and progress to supervising RN or physician.

  • Assist patients with activities of daily living (ADLs) such as bathing, dressing, eating, mobility, and toileting when needed.

  • Support patient rehabilitation programs and carry out care plans as directed.

  • Collect lab samples, perform basic diagnostic tests, and assist with medical procedures.

  • Ensure infection control practices are followed at all times.

  • Maintain accurate and timely patient records in compliance with facility standards and HIPAA regulations.

  • Provide education and emotional support to patients and families regarding treatment and care.

  • Supervise and guide CNAs and HHAs in providing high-quality care.

  • Respond quickly and effectively to patient needs and emergency situations.

Qualifications

  • Current Florida LPN license in good standing.

  • CPR and Basic Life Support (BLS) certification (current and valid).

  • Negative TB test or chest x-ray (per state requirements).

  • Level II background screening (per Florida Agency for Health Care Administration).

  • Strong clinical judgment, communication, and organizational skills.

  • Compassionate, reliable, and committed to delivering excellent patient care.

Working Conditions

  • Work performed in patient homes or residential care facilities.

  • Requires physical ability to assist patients with transfers, mobility, and daily living needs.

  • Flexible scheduling available (days, evenings, weekends, holidays).
